Conscious Human Intention in the Workplace

You are invited to participate in an academic research survey as part of a study on the effects of conscious human intention and its effect on improvement initiatives in the Canadian workplace. To describe it more at this point may influence your survey answers, but we do suggest that whenever you are faced with a survey question that asks for your views or feelings, you let your intuition guide you. The research is being conducted as part of an Business Masters Thesis through the University of Leicester. The survey will be open during November 2014 and results of the completed study will be made available to survey participants in early 2015. All participants will be re-directed to a Thank You page, with a link to the BONUS ISO 9001 DIS document, after completion of the survey.

In this survey, you will be asked general questions about the organization you work for and your position as it relates to the last complete improvement initiative you were involved in. Further questions will be related to your knowledge, experience and opinions of the last improvement initiative you were a part of, how it was conducted, and its level of success. Please take the time to collect pertinent information about your last improvement initiative and reflect on it before taking the survey - It should take approximately 15 minutes to complete the questionnaire if you have prepared.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with completing this survey and you will not be required to provide any personal information, unless you wish to be contacted. However, if you feel uncomfortable answering any of the questions, you can withdraw from the survey at any point.

Your survey responses will be kept strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
